Primary Duties and Responsibilities
o Contact families coming through the Front Door to provide information and guidance on the process. o Conduct Front Door Trainings for individuals and families.? o Conduct intakes/assessments with individuals and families to determine appropriate service level. o Complete Developmental Disabilities Profile 2 forms. o Open EAA tool and enter identifying information. o Meet with families and other key stakeholders to gather pertinent information and documentation. Process the information and submit service recommendations for authorizations. o Track activities to ensure the timely progression of individuals through the Front Door process. o Keep families aprised of progress through the Front Door process. o Direct families to the Coordinated Assessment Process o Approve Quality Review Team(QRT) 1 requests and submit QRT 2 requests for approval, when necessary. o Review Home and Community Based Services (HCBS) packets before submitting to HCBS unit. o Process Notice of Decisions. o Process Level of Care Eligibility Determinations. o Close EAA tool and engage in lost contact protocol, when necessary. o Troubleshoot problems/issues with the Front Door Team. o Participate in weekly Front Door team meetings and training, as scheduled. o Participate in functions and activities occasionally, during evening hours and weekends. o Complete mandatory training. o Participate in special projects as assigned by supervisor. o Duties and/or caseloads my change based on operational needs. SELECTION CRITERIA:
o Possess excellent and professional communication, public speaking, written and organizational skills. o Ability to initiate and maintain working relationships with individuals we serve, families, advocates, voluntary agency staff and Non-Medicaid Service Coordination and Care Coordination Organization staff. o Working knowledge and proficient use of a computer. o Ability to work independently and with a team. o Possess acceptable time and attendance work record

Salary Range:
From $60069 to $76387 Annually
Minimum Qualification
MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
o Currently serving as a LMSW 1 (Spanish Speaking) and eligible for transfer OR o Reachable on the Civil Service List for Licensed Master Social Worker 1 (Spanish Language) OR o Eligible for 70.1 transfer per Civil Service Rules and Regulations. MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT:
o Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
