o Provide clinical and administrative oversight for environmental modifications (EMOD), vehicular modifications (VMOD) and assistive technology (AT) projects for those in the Home and Community Based Services (HCBS) Waiver, o Review Life Plans, Level of Care Eligibility Determination (LCED), medical/clinical justification, proof of home/van ownership and other pertinent documents to determine eligibility for EMODS/VMODS/AT projects, o Correspondence with care managers (CM's) to obtain to obtain necessary or corrected documents, o Maintain EMOD/VMOD/AT tracker and data base, o Present information to CM's and care manager supervisors (CMS's) at monthly/quarterly CM and CMS meetings, o Plan and/or assist with meetings with voluntary agencies contracted for EMOD/VMOD/AT work, o Troubleshoot issues that may arise during EMOD/VMOD/AT review process, o Prepare and present trainings on EMOD/VMOD/AT process to other LIDDRO units, as needed, o Written and verbal correspondence with Central Office, voluntary agencies, CM's, families and the LIDDRO Fiscal unit during review process, o Issue and send NOD for EMOD/VMOD/AT requests, o Participate in EMOD/VMOD/AT fair hearings, o Receive supervision from HCBS Waiver Director, o Attend HCBS Waiver team meetings, o Special assignments/projects as assigned by Administration

Salary Range:
From $61270 to $77912 Annually
Minimum Qualification
MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
o Currently serving as a LMSW 1 (Spanish Speaking) and eligible for transfer OR o Reachable on the Civil Service List for Licensed Master Social Worker 1 (Spanish Language) OR o Eligible for 70.1 transfer per Civil Service Rules and Regulations. MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT:
o Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).Estimated Salary: $20 to $28 per hour based on qualifications.
