o Work with individuals transitioning from residential schools to adult services with OPWDD. o Perform Front Door assessment/EAA tool for individuals who are transitioning out school into adult services with OPWDD. o Work with school district personnel in identifying upcoming graduates, scheduling Front Door sessions and assisting in transition planning. o Track and report data on each graduating class, work with families in the transition planning process and document communications. o Work closely with provider agencies to develop transition plans o Maintain dialog with families, both in writing and by telephone, regarding their child and the placement process. o Attend meetings with school district personnel, voluntary providers, advocacy organizations and other state agencies as required. Selection Criteria:
o Possess excellent and professional communication, public speaking, writing and organizational skills. o Ability to initiate and maintain working relationships with individuals served, families, advocates, community representatives, voluntary providers, DDRO and DDSOO agency staff. o Computer knowledge and ability to maintain databases and monitoring systems. o Ability to work independently and as part of a team as required, as well as provide direct supervision to unit staff. o Possess acceptable time and attendance work record. o Ability to participate in requested functions and activities during evening hours and weekends as requested.

Salary Range:
From $61270 to $77912 Annually
Minimum Qualification
MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
o Currently serving as a LMSW 1 (Spanish Speaking) and eligible for transfer OR o Reachable on the Civil Service List for Licensed Master Social Worker 1 (Spanish Language) OR o Eligible for 70.1 transfer per Civil Service Rules and Regulations. MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT:
o Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).Estimated Salary: $20 to $28 per hour based on qualifications.
