o The Crisis Liaison is responsible for individuals in need of immediate services to avoid an out of home placement. Work with individuals, families, Care Managers (CMs), and other stake holders who come in through the Front Door and currently in need for immediate services. Consults and interfaces with hospitals, APS/CPS, school districts, families, and provider agencies with complex cases to facilitate appropriate options within Family Support Services. Responsible for intakes for special populations, crisis calls, and correspondence. The person will work with families in crisis as well as individuals who are living at home and require support services to ensure the health and safety of the family. Good problem solving skills are required. o The Crisis Liaison will be responsible for conducting intake assessments for incoming crisis inquiries. Crisis Liaison will need to provide initial recommendations and referrals to address immediate crisis situations. Crisis Liaison will be responsible for reviewing Individual documents, identifying missing information in crisis referral, and providing concise updates to Crisis Supervisor. o Will ensure that individuals with complex service needs can secure adequate treatment options when they need them most in the least restrictive setting possible. o Will ensure that individuals at risk of hurting themselves or others will be engaged in services and have non-hospital based emergency care available when needed most. o Works as a member of the team but must also be able to independently determine disposition of a case, which often times requires independence of direction based on knowledge of available resources. o Crisis Liaisons must be able to work occasional evening and weekend days. o Field work, which may involve home visits, outside meetings with CMs, private agencies, and families, etc., strong computer skills, and ability to prepare accurate and timely written records and reports are a must. o Special projects as assigned by the Administrative staff.

Salary Range:
From $56604 to $71980 Annually
Minimum Qualification
MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
o Currently serving as a LMSW 1 and eligible for transfer OR o Reachable on the Civil Service List for Licensed Master Social Worker 1 OR o Eligible for 70.1 transfer per Civil Service Rules and Regulations. MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT:
o Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
